Before Your Visit

Before you visit one of our offices, please make sure you bring a photo ID and your insurance card.  It is your responsibility to double check that our doctors are in your insurance network prior to visiting our clinics.  (Please see our insurance page.)  

Additionally, a proper well care exam requires a complete examination of every part of the body so please have your child dress accordingly. Many older kids are more comfortable wearing their gym clothes or even their bathing suit and with some sweatpants/sweatshirts over that attire – it makes it easy to be ready for the check-up.
